Changes in OCTA parameters of choroidal structure after scleral buckling

Abstract: Purpose: To evaluate changes in the OCTA parameters of choroidal structure after scleral buckling (SB) for retinal detachment repair. Introduction: Scleral buckle has been known as a well-established treatment of rhegmatogenous retinal detachment (RRD) for many years. This procedure is associated with structural and vascular changes in the choroid. Previous studies have shown that it can lead to choroidal stasis and congestion which secondarily increases the Sub-Foveal Choroidal Thickness (SFCT).

Methods: Method: This retrospective observational case series studied 30 eyes from 30 patients with an unilateral RRD that had been repaired successfully with a SB surgery (segmental or encircling) at Labbafinejad Hospital in 2017 and 2018. There were 2 control groups first group consisted of 30 fellow eyes and the second group included 30 healthy eyes. Cases and control groups underwent EDI- OCT before and after surgery and the OCT parameters were analyzed with image-J software. Demographic features, ophthalmic evaluation, choroidal thickness, and vascularity parameters were compared pre-and post-operatively between cases and two control groups.

Results: Result: There was no significant difference between preoperative and 1 year postoperatively in the SFCT between cases and controls (p value 0.68). In the OCTA vascular parameters, There were significant differences between luminal area/total choridal area[choroidal vascular index (CVI)] before and after surgery,and between cases and controls.(pvalue 0.010).

Conclusion: Conclusion: SB has a considerable impact on the choroidal vascular parameters (CVI) after one year. SB increases CVI.
